Firearms Activity Waiver & Consent Form
Thank you for your interest in participating in activities at Hermit Lakes Recreation Incorporated.
Before participating in any shooting range activities, all participants must complete and submit a signed waiver form. Participants under the age of 18 must have a parent or legal guardian complete and sign the waiver on their behalf. Adult participants (18 years of age or older) may complete and sign the waiver themselves.
Please review all information carefully before submitting. By signing and submitting this form, you acknowledge that you have read, understand, and agree to the terms outlined below.
You may submit your waiver in one of the following ways:
Online: Complete all required fields and click the Submit button at the bottom of the form.
Paper Copy: Download and print the waiver, complete all required information, and submit the signed form to the Hermit Lakes Recreation front office before participating in any shooting activities.
All waivers must be received prior to participation in shooting range activities.
